WYLDLIFE SUMMER CAMP

STUDENTS THAT HAVE FINISHED 6th - 8th GRADE
What to expect:
- Transportation to/from Crooked Creek Ranch
- An amazing time with your friends!
- All meals are included while you are at camp (you can bring money for the store/snack bar at camp)
- All camp activities and life changing experiences
Final payments due by 7/1/24.
REQUIRED:
- ONLINE HEALTH FORM - Complete form online, upload immunizations, and print physician signature form. Our area number is CO135.
- PHYSICIAN SIGNATURE FORM - Take printed physician signature form to your child's physician to complete and fax to YL. You may want to pick up a copy to keep for your records. (We will not accept paper immunization records or paper physician forms due to HIPAA). WE WILL NOT ACCEPT SCHOOL PHYSICAL FORMS!! Failure to submit the correct signature form will result in a $100 fee for your camper to see the camp doctor to have the form completed.

Students that have finished 9th-12th grade
What to expect:
Airfare and bus transportation to/from camp.
- An amazing six days at one of Young Life's premier camping properties
- All meals are included while you are at camp (you can bring money for the store/snack bar at camp)
- All camp activities and life changing experiences
Final payments due by 7/1/24. Full payment due upon registration for campers registering after 7/1/24.
Cost: $999
REQUIRED FORMS:
- VALID PASSPORT - Passport should not expire within 6 months of camp trip
- PARENTAL CONSENT FOR BORDER - Print, sign, and bring original, signed copy to your YL leader by July 6th.
- NO-TOLERANCE AGREEMENT - Print, parent & camper sign, and bring to your YL leader by July 6th.
- ONLINE HEALTH FORM - Complete online. Our area number is CO135. Use allisonawyatt@gmail.com as the YL Staff/Trip Leader email please.
Campers may bring ONE checked bag and ONE carry on.
